24 HOUR CANCELLATION/RESCHEDULING POLICY
We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, the following policies are honored: 24 hour advance notice is required when cancelling and/or rescheduling an appointment. This allows the opportunity for someone else to schedule an appointment. Massage Therapists earn their living for themselves and their family based on clients honoring their appointment obligations. If you are unable to provide 24 hours advanced notice upon cancelling and/or rescheduling your appointment, you will be charged a $25.00 late cancellation fee by the local Massage Advantage Center that your appointment was scheduled with. For clients who have cancelled or rescheduled less than 24 hours in advance for multiple appointments, a $25.00 late cancellation fee will be applied for each appointment.
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a no-show and liable under the 24 Hour Cancellation Policy. The client will be charged a $25.00 late cancellation fee by the local Massage Advantage Center that your appointment was scheduled with, even if the client secured the appointment by purchasing a voucher code from an outside company. For clients who have no-showed multiple appointments, a $25.00 late cancellation fee will be applied for each appointment.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the session. Out of respect and consideration to your therapist and other customers, Please plan accordingly and be on time. We look forward to serving you!
If in the course of your massage you act inappropriately or in any suggestive manner your therapist will immediately terminate the session and you will be expected to pay full price for that session. Failure to do so will result in us notifying the authorities of your actions. You will also be banned from making any further appointments.
GIFT CERTIFICATES/GIFT CARDS/MEMBER MASSAGES:
Gift certificates, gift cards, prepaid membership massages are not redeemable for cash.
We cannot accept or verify lost gift certificates and gift cards.
All gift certificates and gift cards must be presented at time of service or you must pay by other means.
All gift certificates and gift cards are non-refundable.
Gift cards are available for purchase in a.) any dollar amount and may be used to purchase services at participating Massage Advantage centers and b.) units of massage. Cards with massage units hold blocks of money that cannot be divided; these cards are redeemable for massage at the rate paid for at the time of purchase. In the event of a price change, the user is responsible for the increase in rates. All gift cards have no expiration date. It is the policy of Massage Advantage to not issue refunds for gift card purchases.
MEMBER MASSAGE CUSTOMER INFORMATION
Memberships are good for one year from the purchase date. Massage Advantage members are entitled to a discounted price on regular, full-price massages. Any services not used in full by the end of the one-year membership term will be considered expired and no refunds will be given.
Memberships can be canceled before midnight of the 3rd business-day following contract activation with no penalty (less the value of any services received). To cancel membership, you may do so via email to: Info@MassageAdvantage.com OR a signed and dated written notice of cancellation must be sent by Registered Mail with Return Receipt Requested to: Massage Advantage, Inc. P. O. Box 8167, Jacksonville, AR 72078.
If the membership is cancelled at any time following the 3rd business-day after contract activation, a cancellation fee of $99.00 will be deducted from any remaining balance on the account. All refund requests for less than the amount of the cancelation fee will be denied. Refund requests must be made in writing by Registered Mail with Return Receipt Requested to: Massage Advantage, Inc., P.O. Box 8167, Jacksonville, AR 72078.
Advantage PLUS Member (APM) CUSTOMER INFORMATION
Advantage PLUS Members are entitled to purchase unlimited one-hour massages for $39.99 and half-hour massages for $29.99 which may be redeemed at any time while the Advantage PLUS Membership is active. The initial contract period for membership is twelve months. APM massages may be redeemed so long as Advantage PLUS Membership is PAID current and ACTIVE. Upon termination or cancellation of your membership, all unredeemed APM massages will expire.
Advantage PLUS Membership is auto-renewable. Following the initial term, the membership will automatically continue on a month-to-month basis until the membership is cancelled. All cancellations require a 30 day written notice via email to APM@MassageAdvantage.com OR A SIGNED AND DATED WRITTEN NOTICE OF CANCELLATION MUST BE SENT BY REGISTERED MAIL WITH RETURN RECEIPT REQUESTED TO MASSAGE ADVANTAGE, INC. P.O. BOX 8167, JACKSONVILLE, AR 72078. All cancellations are effective 30 days after the date received. Payments due prior to the effective date will be charged as scheduled.
Billing for Monthly Dues: Upon submitting this agreement, Massage Advantage is authorized to bill the listed credit/debit card for monthly dues, fees, and any applicable sales tax. A service charge $35 will be applied for each month in which dues are returned uncollectable. The first automatic payment will be drafted one month from the day of contract activation, and will be drafted on the same day each month thereafter.
Default and Late Payments: A default occurs when any payment due under this agreement is more than 5 days late. Should any monthly payment become more than ten days past due, the account will be charged a late fee of $15.00. An additional service fee of $35.00 will be assessed for any credit/debit card returned for insufficient funds or any other reason. For all members paying monthly dues with electronic funds transfer, Massage Advantage reserves the right to draft all amounts owed, including but not limited to any and all late fees and service fees. Reactivating a membership that has been cancelled by the Massage Advantage due to an unpaid balance will require the payment of the unpaid balance. If the account becomes more than 5 days past due, all APM massage will be considered void.
DISCLAIMER OF LIABILITY
Massage Advantage Center locations work only with professional massage therapists who comply with state, city and/or local licensing requirements. If you would like to see a particular massage therapists’ license or registration, please contact the center administrator. Additionally, if you have any questions, comments or complaints about your massage therapist, please bring this to the attention of the center administrator immediately or Massage Advantage Corporate at Info@MassageAdvantage.com. Except where prohibited by law, you agree that Massage Advantage, Inc. will not be liable for any injury, including, without limitation, personal, bodily, or mental injury, economic loss or any damage to you resulting from negligence, other acts of the center, anyone on the location’s behalf, or anyone using the services of the facilities of the center.
Refunds on massage are extended for three day beyond the original purchase date to the original purchaser, unless service has already been scheduled with facility.
Purchasers request for refund must be addressed to the location of initial purchase in writing Cash refunds are not available for Gift Certificates, Gift Cards, or Membership Massages or any other service beyond 72 hours or original purchase.
